Plenty of people on track bikes, racing on the track, use clipless. The points race, madison, team pursuit...
I don't think I've ever seen anyone reach out with a foot to try to break a fall. And I've seen dozens of crashes, from the perspective of a spectator, racer, or being involved in them directly.
And yes, if one foot comes out in a crash with toe clips, the other leg, the one still attached to the bike, is going to pay the price.
