Can't get cleat back far enough

I have my Keo cleat pushed as far back on my new DMT as possible and it seems like it's still in front of the ball of my foot. When I climb or stand in the saddle I feel a strain through the arch of my foot and quads like I'm pedaling on my toes. A recent fitting confirmed that the ideal cleat position should be 10-15 mm further back. I had the same problem with my old Sidis.

I only have this problem in the left foot. What can I do? I'm out $600 after fitting and shoes with no improvement!
