Quite often I position my hands on the tops of the bars as close to the stem as I can. It's actually quite aerodynamic. The body is positioned higher, but the arms tuck in very narrow. I've thought, at times, of mounting some mtb bar ends up close to my stem so I could stretch out a little in that position.

Of course, this position has a few control and climbing issues, but I commute on some of the flattest and straightest roads in California so I don't really care.
