Small chance my problem was the same as yours, but... When I first set up my fixed gear, the lockring was getting loose. Turns out I put the Formula brand lockring on backwards. I think, due to its design, the lockring was not able to snug up against the cog tight enough before reaching the end of its threads, so it was susceptible to coming loose.
