If I used a strong "scraping" motion on the upstroke, my foot would often slip out.

If it was a matter of heel control, I was never able to get to the point where I could control it
This should not happen -- it is impossible for me to get my foot free using the movement you are describing. When you clip in, you should feel and hear a click that locks you in. The only way to experience what you are is for the clip not to engage. I'm wondering if your cleats are defective or possibly not installed correctly.

The cleat is in two pieces. There's the part that the bolts go through, but there's also a separate part between that piece and the shoe that should have a piece of stuff that feels kind of like rubber or styrofoam on it. If the rubbery thing has been pulled off, your cleats will not lock properly.

Are you feeling the click, and if not, have you tried rotating your foot around a bit as you put force on the downstroke?
