Insurance while riding?

I ride a motorcycle and drive a car in addition to riding a road bike. With the car and motorcycle, if someone hits me, their insurance will cover my vehicle damage and healthcare costs. If they don't have insurance, my uninsured/underinsured coverage will cover my own vehicle damage and health costs.

With the bicycle, what's the deal? I have typical health insurance that will cover any health costs. But if some idiot in a car pulls out in front of me and sends me over the handlebars and I break an arm, what happens? Do I make a claim against his vehicle insurance?

And what if the driver doesn't have insurance? My health will be covered, but what about the bike and my out-of-pocket health costs?

Is there such a thing as bike insurance that covers riders in the same way typical car insurance coverage covers drivers?
