I dug around a few months ago, and though I did not need another bike, I found that I liked the Civia listed here, with MSRPs (also mentioned above by alev). I saw a LBS using the Civia Alfine build model as rentals, and when I asked them why, they simply said 'less maintenance'.

Koga Miyata makes a nice handmade bikes like you are looking for, but their models I am thinking of mostly come with Magura hydraulic rim brakes or roller brakes instead of discs -- see here and here -- and Kogas are available in the USA at Commuter Bicycles in Santa Barbara, CA, but the weak dollar is making it tough for importers to compete (but they do).
