I've fallen off my trainer a few times, never seriously (my teammate in high school put his hand through a glass full of water though). Usually it's when I'm watching a tape of pros descending and I start imitating their cornering technique.

What exactly happened in your original crash? Also, are there similarities or triggers that make you "flash back"?
On a side/related note, have you ever done bumping or wheel touching drills? And have you ever taken a martial arts or gymnastics type class where you have to learn how to tumble/fall?
