mebbe not tight enough if its creaking. sure its the stem-area creaking and not your bb, headset, etc?

try to disassemble, check for any surfaces worn shiny on bar and stem, sand lightly with medium grit if there are, clean thoroughly, and then tightly reassemble. has worked for creaking ive had in the past. I wouldnt do rubber shim, just personally, that for me is like asking for it to give or shift at high loads.
