The pedal that failed was from my third set - the first I beat up in a couple of crashes - the outer housing was so scraped and worn away that the cap would not stay in - no threads left to hold it.

Set #2 was I bought to use on my old Schwinn LeTour. They are not TI - but I pulled them off the LeTour to use on my TCR until I can order a rebuild kit from Crank (looks to be about $15 in cost).

I'm just surprised at this failure. Set #1 was far more abused and seemed to keep on going.
