Frame builders have a variety of dropouts available to them, so I would think that it might be possible to replace just one in many cases and match it reasonably well, unless the dropouts are really unique.
My LBS web site shows a price of $75 for dropout replacement (one of the co-owners is a frame builder) and I don't know if that counts the cost of the dropout, or just the labor/brazing etc.
I'd like to believe that's the price to replace it complete.
