Seems to me that this complexity should be resolved out of sight of the customer, and in the customer's behalf. That's the point of the FTC rule, I think. If there is a communication problem, it's the seller's responsibility to be proactive about refunding money and contacting customers. After 30 days, if the customer hasn't approved a delay, the transaction has been cancelled.
