You're doctor either doesn't know what he's talking about, or you don't remember what he said.
A pound is a measure of weight, not impact.
If you dropped a 4 lb weight from the empire state building, it could break most anything.
If you dropped it from a foot above your collar bone, you'd be fine.
Impact energy is expressed in terms of kinetic energy. 4 lbs of force on any bone is not going to hurt anything.
