With 120mm dropout spacing, you can mount 5-speed internal gear hubs from Sram and Sturmey-Archer, in either Drum-brake or Coaster-brake variants.

If you don't need hub brakes, Shimano Nexus can go as small as 123mm and Sturmey 8 can go to 116mm.
