Was the hub overhauled by the LBS, by you or was it brand new before the wheel build?

If the LBS overhauled the hub and did it wrong, you have a very good claim. If the hub was defective when they got it, the fault is less clear. They should have detected it but the off-spec hub could have contributed to the miss-dish.

There is NO excuse for the excess tension and stripped nipples.
