I'm not sure of the saddle height issue, but I think you need to adjust your hand and arm position (which may or may not involve switching out your handlebars). Looks like your elbows are bowing out a bit in more of an MTB posture, and don't have enough bend in them. Elbows should be pointed almost straight down to the ground and have as much as a 90* bend in them, though normally less.

If you get positioned this way your hands should align such that the "V" formed by your thumb and hand more or less is centered on the hood/drops, and you'll remove pressure from your ulnar nerve where it exits your wrist and enters the palm. You should only be leaning on the handlebars with very light pressure. By doing this adjustment you should relieve pressure that causes numbness in the hands. It will also cause your shoulder blades to drop a bit down your back, reducing some of the hunching of your shoulders towards your neck, which will relieve pressure there and reduce neck pain. An added bonus: you'll get in a more aero position.

You may need to go to wider bars to do this, or just concentrate on your form. You may also have to work on your core strength to better support your upper body, or possibly move your saddle back a bit to get more of your weight back, but I'd hesitate to do this adjustment given your knee condition.



No idea what might be causing the toe issue.
