FWIW: For the RR and TT portion of our stage race we use Volunteer Fire Departments - don't know if you have any of those around. We donate liberally to them. Around here they are very helpful and well trained. Three years ago they saved a racer's life as they had just received their portable defibrillators. In town for the crit we pay to have a city ambulance stand by.
If you want to see a really difficult medical problem, look at a 63 mile backcountry, mostly single track MTB race. Makes medical for a RR look REAL easy.
