The mounting bolt is tight. It's the adjusting screw that backs itself out.

I guess I'll just get some blue loc-tite and try that.

Now that I look at it the cable is a little long. I never shortened it when I flipped my stem, but it was doing this before I changed that. Could that be doing it?
