I wear size 15 wide shoes so I use the XL Power Grips, but I have them on the tightest setting for my usual riding. If I have to wear some heavy winter boots, I open them up a few notches.
It's not the Power Grips that cause the pain, it is the mashing. I have them as tight as I can get them and still be able to get my foot on the pedal. The slippage is not a lot. It isn't like my foot comes completely off the pedals. I have never measured it, but if I had to guess, my foot would slip 1/4 to 1/2 of an inch. Just enough where I would need to reposition my foot. It could be the pedals also. If I got some quills with a good grip, there probably wouldn't be as much slippage. But then I still wouldn't ride it far in soft sole shoes.

I do like Power Grips, I have them on one of my bikes and I don't see a reason to change them. But that bike is for shorter trips where I can mash and not cause pain.
