Just a comment or two...
Rivendell fits people on a very large frame. Consider their recommendation as the absolute maximum. I prefer about 2-3 sizes smaller, but I like small frames and ride with two fists of seat post showing. I am about your size and tour on a 49 cm frame, but I like an aggressive position with the bars maybe 4" below the saddle. I would advise definitely going smaller than 54 especially with an LHT.
I am not a Rivendell fan, and recommend taking what you read there with a larger grain of salt than usual.
