Never had anything fall out, even in a crash. You're over thinking it, if you're over stuffing them that much you're either over packing or you need frame bags. I went out this weekend with a vest, cell phone, credit card, gel flask, a bottle of perpetuem for later (small bottle w/just powder), and 2 banans and nothing fell.
+1 I'd even go so far as to say stuff is even safer when your pockets are stuffed. It stretches the fabric and makes everything tight, holding it in place.
