The Milano 3V is an Italian built bike from Alberto Masi.

You need a threadless headset and stem.

PLEASE do not drill into the frame it wont work because the tubing is VERY thin. You would need to have bosses brazed on which will destroy the paint (which is brutal imo so maybe not a big loss..) Use bar end shifters if you don't want to use brifters.
