How do you managed on times when you really need to relieve yourself badly and you can't hold it for the next toilet that is 15km away?
well i was in those shoes a few days ago, i was having too much fun exploring hidden forest in the lower skirt of the civilization, and i was unable to find a way out, road around for about an hour and i was still sort of lost inside the forest, at that point i just couldn't hold it any more, so i park the bike by a nice old tree with very tall roots to keep the bike from rolling down the cliff, then proceed to find another tree to relieve myself.
After that, i was no longer under pressure, and i was able to continue enjoying my forest exploration
I think it is even more a problem when road riding and you're still in civilization and more likely to be caught. Here in Mass you could run the risk of being a sex offender by exposing yourself.
When I am riding out in exurbia, when the urge usually hits after riding for an hour or two from out of downtown, I feel pretty comfortable in going to a fast food joint; McDonald's and Dunkin Donuts are ubiqitous, even out in the sticks. I don't carry a lock, but I think being out in sub-exurbia where car is king, patrons of the restaurants are less likely to be interested in a bike. The also have vestibules to bring the bike in. I move quickly and have never been hassled. Sometime I buy an iced coffee to drink quickly and legitimize my visit.
