Has this ever happened to your clipless pedals?

This happened about mile 65 of a century ride on Sunday. I went to clip back in, and the jaws (or whatever they called were open). I was able to fix it by wedging an allen key on one side and tilting the jaw with a flathead screwdriver. The other 35 miles were fine.

The question, is that "wife" permitting I'm thinking of doing a double-metric this coming Sunday. It this something to be concerned about or does this occasionally happen with a forceful unclip? Tension, springs, etc, look fine.
