Car free and the extended family

My family is car-light, not car-free. For one, we have an infant and I like knowing I can get her to the doctor or wherever she needs to be quickly and easily (especially in the Canadian winter). The other reason it is nice to have a car is so we can visit family. Most of my wife's family lives about an 1hr's drive away and the rest is about a 4hr drive away. My side of the family is a 4hr plane ride so that is mostly irrelevant to this conversation.

My question is to the truly car-free. How do you deal with extended family? I'm guessing a lot of people rely on their relatives coming to visit them, they live close enough to bike for visits, or they simply don't visit much.
