chris, those allens are tight. the shifter itself is not the problem. this is just the plastic cap and the friction/index "d-ring" rattling.
Sounds like your assembly is correct, and I wouldn't think that the RD or FD would have be readjusted. My right came loose mid-century once and had to disassemble and tighten the compression part to keep it in the bars. No other adjustment necessary.
I'm assuming the plastic ring is more for looks, so maybe a drop or two of super glue? I can check out mine later to see what it looks like exactly.
