for sure adding a brake (sidepull at least) should be possible? remember the one sixty posted here some weeks ago?
Not sure what you mean about the back pedaling. Does she want to mowe the bike forwards by pedaling backwards or does she just want to backpedal when coasting as you can do on a der bike?
Guess the drivetrain can be put on the oposit side if there is good reasons, but be sure there is. Unless there are some physical reasons for it just put on your headphones ', turn up the volume and wait for her to get used to it.
