Left one is the basic barrell. Right one is the lever stop.

They are the Dura Ace stops, sell for a little more, like $20 vs. $10 for the straight barrrel jobs.

The lever provides more cable tension with the flick of it, probably has 3 "indented" positions. I've never moved either one. I guess some cables stretch, or something, while riding.
