Admittedly, the report could be incorrect, but based on the witness' account recited in the report, the cyclist tried to pass the Truck on the right, in the truck's blind spot as the truck made a legal right hand turn.

Other than not stopping, which may have been the result of not even feeling or hearing the collision, there is nothing to criticize the truck driver for from the available information.
First of all, how terrible for everybody involved. As a truck driver, I thank you for the insightful, intelligent response. He would never know he hit somebody on bike, hell, even hitting a car is not that big an impact to a vehicle that weighs in the neighborhood of 50,000 lbs. As a truck driver, I have this problem with cars as well (driving in the blind spots especially on the passenger side). The fact of the matter is that even the best designed truck cab has blind spots, it is the nature of the beast. Anytime you are near a truck you have to assume the driver cannot see you, so don't hang out next to it. I have long maintained that going for a ride/drive in a large truck should be mandatory before getting a driver's license, too many klingons driving around the highways right now.
