I used in-line adjusters on my 2001 Cannondale, which only had brazed on cable stops for the FD & RD. I am running a Campagnolo Comp Triple groupo, and while the adjusters were supposedly for Shimano gear, they work perfectly. I got mine at the LBS, but you can order them any number of places. I believe this is the same sort of gadget BigBossMan cites.
