Carnac Quartz. Bought my first pair in fall of 2003. Still using them, although they may have to be retired when spring rolls around. Flat out the most comfortable cycling shoes I've ever worn.

They're heavy (by cycling shoe standards) but right out of the box they fit snug without pain, and I could go for a 4 hour ride without knowing I had brand new shoes on. I've got one more brand new pair in a drawer at home. Once I wear those out, I'll have to find something new, but that shouldn't be for another 4-6 years.

Near as I can tell, Carnac shoes are very difficult to find in North America these days.
