Disc brake adaptor for AMP B4F4

Trying to upgrade my AMP B4F4 to Avid's disc brakes...currently using Avid's Ti V brakes...unfortunatially my AMP is pre-international standard on mounting brackets. Various other AMP owners have made theirs...some have referred me to HammerHead and they may still have some...but maybe there are other choices/options. Anyone have any ideas...be kind!

This AMP is currently at 22 lbs 6 oz...also upgrading to American Cassis wheelset and with disc's should still lose about 4 oz's...expect 22-4....also using Stan's NoTubes with Hutchinson Python Lites....
