Technically not a bicycle...
My grandfather built this in the 30s or 40s. He machined the 4 cylinder engine in his garage shop. I rode it when I was a kid. It recently came into my possession. Grandpa was the guy that told me, "riding on 2 wheels is like flying on the ground."

The drive system is a v belt. The cover behind the seat hides the transfer pullies. The rear wheel pully is almost the size of the rear rim.

My late father-in-law (late wife's dad) was the kind of guy to do this, too, although the scratch built motors he made were just to be bench mounted and run for the sheer artistry of it.

Four cylinder engine, the style of the bike is a direct take-off of the Henderson, Cleveland and Ace (later Indian) 4. The way he did the tank is a copy of those bike's. Beautiful work.
