Anyone ever break a Miyata Mangalight fork?

The position of the crack is such that braking forces would aggravate it, but I have no idea how it could have gotten started. Do these forks have a record of breaking that I'm not aware of? I've never heard of such a thing, but I wonder if anyone else has broken one of these, and if so, how did you do it?

Mangalloy can be brittle if the manganese content isn't high enough. Not saying that's what happened, but it is the first thing I'd look at.
