You should be able to tell if this is a problem by there the puncture is. If the puncture is on the inside (rim side) of the tube, it's coming from the rim somehow. If it's on the outside, it's not.

Hi folks. Been absent for a while. Im still not on the bike as I have not had time to fix the tire.
A couple notes:
My tires are new Ice Spikers, bran new (well they've been on for a few months)
I never found what punctured the tire and there was no break in the tire (where the tube had its flat)
I checked the rim tape as well and it is seated just fine.
I did NOT check for another hole, as I think that would be rare.
Im assuming I didn't patch it up well...in any event, I will re-check the tube this weekend in a bucket of water and see where the problem is. I will check the entire tube. I will say though, those Ice spikers are a b***h to put back on... that last little part, man did my fingers hurt afterwards.
I will post later this weekend with an update.
I am not removing the tire as it is real, real hard to put back on. If I only I could find a way to permanently pull my forks apart a bit, that would resolve the problem.

Pinch puncture maybe? From Sheldon:

A hole in an inner tube caused by getting the tube pinched between the rim and a hard, sharp object, such as a rock, curbstone or the edge of a pothole.
Pinch flats (also known as "snakebites" because there are usually two small holes in the tube, as if made by the fangs of a snake) are usually caused by carelessness or riding under-inflated tires. They can also be caused by the use of a tire which is too narrow for the weight it is asked to carry.
If the tire is too soft, or too narrow, it is easy for it to bottom out when striking an obstruction.
If the tire is hard enough and plump enough to carry the weight, the air will keep the tube from being pinched between the rim and the road hazard."
