Made a new one last night: got my first flat of the year on the way home from work. Pulled the rear wheel off, opened my tool pocket on my panniers and discovered my spare tube was 26" while my rim is 700c. In the spring I had transfered my panniers from my winter bike to my summer bike, but I neglected to swap the spare tubes. Doh!

I tried shifting my stem shifters at 15 mph while riding with one hand holding the flashlight and the other hand shifting in the middle of the night in an isolated part of campus.
On the bright side, my knees and arms healed within a week and that numbing sprain in my wrists went away shortly thereafter. That and the fact that it had one bald tire and one grippy one meant I crashed quite often and probably contributed to the bent bottom bracket axle.

The only commuting mistake that I can think of, is not commuting by bike.
