I'm always well lit at night -- 2 headlamps and 2 blinking tail lights. Few cars to bother me and I haven't been troubled by stray dogs yet. But deer! I always have to keep my eyes out for them. If one runs across the road in front of me another is sure to be close behind, and they're not careful of bikes. Wondering if those ultrasonic whistles I used to see advertised are of any use.

I ride into work at 4:30AM so it's always dark for my morning ride. I feel much safer in the dark with lights than in the day.

Yep. I ride at night. Two blinkies and two headlights. On very dark narrow rural roads for the most part. I think you stand out more and people are more aware of you than in busy places with lots of traffic and lights. The main thing that concerns me are drunks.

I am legitimately interested to know whether this is a perception, or whether you have encountered actual drunk drivers on the roads. Or are there statistics from the police based on testing or court actions that indicate the incidence of drunks (or these days, drugged) drivers is enough to cause concern?

I regularly ride at night. My choice of headlamp is a Shiningbeam MC-E flashlight in a cheapo holder from DX. On the rear I use two PBSF's, or a SF and my home made super-bright tail light. Only thing I don't like about my home made light is that it uses 4 AA's and they take more work to use than a PBSF. I also have a $10 red led flashlight from DX that uses the same 18650 batteries that my flashlight uses.
My riding is on almost desolate rural roads so I stick out like a sore thumb. Drunks? Yea. Sometimes they curse at me, sometimes they hit their brakes when they come up on my bright lights flashing in the dark. I think they think I'm the heat and they want to turn around and get out of there so they don't get busted for DUI.
Nothing beats a cool, quiet 10-15 mile ride out in farmland on a moonlit night.
