Everest still makes an old school MTB lugset and BB. I built my first MTB frame in 1985 using their lugs and used their BB as recently as 2005 to make an MTB tourer.
What is really difficult to find is the early 80's bottom bracket shell with the straight chainstay ports and 22.2 round S bend chainstays.

In regard to silverbraze's lug cost presentation:

Those numbers are actually better than I thought for the economics of getting lugs made, of course you left off Bottom brackets and fork crowns. The piece price adds up fast into the total up front costs. How comfortable are you that Your tool will not make parts that just appear elsewhere?

it's very unlikely that you are going to find lugs that aren't in a very limited range of angles. The options are really limited to tig or fillet braze. Or making your own lugs as discussed above
