Name this stem

This stem popped up in an older thread, and I'm not having any luck tracking down it's make/model. Anyone recognize it? I'm a newbie and can't post images yet so I've included the picture here:

That's an old QBP (Salsa/Surly/MSW/Dimension) stem, I don't think it's made anymore. Plenty of options for quill stems out there though, are you looking specifically for one with an open faceplate?

Thanks. Yeah I like the open faceplate, but also the finish and how the weld isn't too bulky. I'm hoping if I can figure out what it is I may have some luck on ebay.

Salsa SUL stem. It came in threaded and threadless. It was recalled...or at least some were.

One alternative that I think looks nice is the Factory Five Titan stem. Very clean looking and nice finish in both polished and black depending on what you're looking for.
