I'm just back from a trip to the States
I was astounded to see on the TV adverts for drive through pharmecy. Drive through banks are so common they don't even advertise them. I have never come across either of these in the UK and I hope I never do!
I suspect the phrase "Nobody Walks, Everyone Drives" (taken from one of the numerous Car TV advets) is a very appropriate way of summing up life there.
I now really admire people that commute by bike in the States for going against this kind of brain washing (and brain washing is what it is). Keep it up guys.

I found drive-through banks useful on my bike, it means I don't have to bring it in or anything!
I do find that many of these drive-through stores mean more idling motors while people don't walk the fat butt into the store!
