Bike rack accidents

I've never had a problem with a bike coming off the rack or the whole rack coming off for that matter, but have any of you had any such accidents?

Only accident I ever had was the "forgot the bikes were on the roof" pull into the carport.

Luckily my Mtn bike is built like a tank.

Never done that again.
