Can and does happen. Marc Vivien Foe, the soccer player, had something similar. Foe never made it to the hospital, just dropped on the field mid-game. With Mingheer, apparently he'd been treated/examined for a heart incident last year and the doctors gave him the all clear to keep riding. I guess the poor guy was just destined to live a short life.
