Under seat steering problem

I am currently building a USS for this recumbent. I followed the procedure on this website https://www.ihpva.org/Projects/Practi...g_the_uss.html

The problem is when I rotate the steering, it will eventually come loose. The bolt that holds the BMX steering stem to the frame will come loose. I have even tried a jam nut to solve the problem.

Here's a quick pick of what I am building.

I'm not sure what the instructions are having you do for attaching the stem. If it were me, I would retain a steerer tube, complete with upper and lower bearing races. (Weld a plate to the bottom of a steerer tube to hold the lower bearing race in place and use a regular headset for the tube.) Then the stem would attach normally and the only mod you'd need to do to the stem would be to add that plate for mounting the steering rod.

Did you put dual flat washers around the bolt that holds the BMX steering stem to the frame ? Did you grease the washers, to allow the handlebars to easily rotate about this bolt? that's whay I'd try first.

I also built a USS using a 1/8 in. thick aluminum plate. On my bicycle, however, the fore-aft strut is perpendicular to the front axis with the front wheel pointed forward. I'm guessing your fore-aft strut is orientated at a 45 deg. angle in order to make both front wheels rotate in unison. Did you verify that it does so with no weight on the front wheels?
