Cycled in the Republic of South Africa and the USA. FWIW I've pedaled underground in both places too.
Edit: Costa Rica also; surface only in the Carrillo Beach area under the Coconut palms.

I've biked in El Paso, btw.
Not a separate country, but I have done some semi-exotic cycling on some Chesapeake islands.
Smith, I think it was fixed gear mountain bike owned by the bed and breakfast of a town with a population in the double digits, all watermen as profession (and innkeeper) The chain actually managed to slip off on a non-shifting bike
Fox, a bay foundation center, they rigged up a bike to pull up water for whatever they do with it (hopefully not drinking) and it gets harder as the psi increases

- italy, near gardasee and vulcan Aetna
- spain, during a holidaytrip
- thailand, live here now
- Myanmar/burma, just a few meter when doing the VISA at border
try to do next malaysia, singapure and philippines
after i met a lot of cycles from there took part at last race "Tour of EGAT 2008" in thailand.
