What does it mean when your knees slam into your handle bars?

What does it mean when your knees slam into your handle bars?
First time it's happened today. Riding along, got up out of my saddle to climb a hill and WHAM! my left knee slammed into the lower portion of my handle bar to the left of the stem. Nearly took me down.

Happened to me a couple of times but it was only when I jumped up to sprint from the hoods.

It means your knee is going to hurt for a while.

I was told by my frame builder (Paul Hillbrick) that its normal for riders to be able to knee the handlebars if they are not careful with their balance and position. You could fit a longer stem if your concerned although then you may be stretched out too far.

I can barely touch my knees to my bars when climbing out of the saddle, but I don't have enough reach to actually slam my knees into my bars. That seems about right to me.
