I reckon he must mean the upper terminus of the bolt whose lower terminus is the star nut, in threadless fork systems. What would make it custom? All the diameters are standard. I do see blank ones that could be etched with grandiose visions of dragons and wizards, albeit on a small scale, with a dremel tool.
