Tire Question

I have a small hole in the center of the tread in one of my tires. Does that ruin the tire?

Does the tube bulge out? Can you see cords?

The tread looks good. No cords or showing. I have had a couple of flats and not sure if it related to the hole or not. The tire only has about 400 miles on it.
