If you must know...
Since I've been riding a bike from age 6, I've had plenty of typical cuts and bruises from minor crashes. The worst was a broken arm in third grade after a failed wheelie. Got left hooked by a car 6 years ago. Destroyed bike, but I got away with very minor injuries. 3 years ago got hit by wrong way cyclist around a blind corner on MUP. Bike had front wheel and chain ring ruined. My left leg got a really huge lump on it and still doesn't look or feel right to this day.

My only non-crash injury was something really stupid: I decided that it would be fun to do hill reps on a local hill that tops out around 11%, on a fixed gear running 46-16, after a long winter with barely any riding. My knee wasn't so happy about that, and I ended up limping around for a few weeks (while I put some lower gearing on my bike and stuck to flat rides for a bit!)
My worst overall injury was the time I got left hooked by a minivan. I had crap grad student insurance and couldn't afford x-rays, but I'm pretty sure I cracked some ribs (hurt to cough for over six months), and half of my rear end turned purple and swelled to double its size. It was nearly impossible to sit down for about two weeks.
About two weeks before that, I had a bike related "incident" - I was trying to carry my bike up some steps, tripped, and slammed the stem into my sunglasses which proceeded to cut my forehead open. This area of course bleeds like crazy, so it looked gory as I bled all over the pavement. I got stitches, and ended up with what I call my Charlie Manson scar.
More recently, I had a low speed crash into a ditch. Wrenched a muscle in my back pretty good, but otherwise fine. The part that sucked is that I was just under 60 miles into a planned 130 mile ride (which I finished, though I was done with sprints for the day.)

Over the handle bars in October 2015 and tweaked my neck enough so that I was off the bike for three or four months.
But I was back on a trainer, albeit moving slowly, within a couple weeks.
