It's called a pursuit bike. The front wheel is a 650c and I'm sure it's an original fork. There are/were many frame makers but I recall Yamaguchi making many such bikes.
Because the rear end is not a track end I am not sure if it is considered a pursuit, TT, or funny bike. I don't see a derailleur hanger.
