I use 48x16. It's a bit taller than it needs to be and I have considered dropping to 48x18, but my commute has no lights at all and very few points where I even have to slow down much.
If I was brakeless, I'd definitely use an easier gearing.

My first conversion project: cheap Peugeot frame to ridiculously expensive bike
Second project: Raleigh 3-speed
Sold to a coworker so he could cruise around town. Heavy-riding bike but fine on the flats.
Third project: singlespeed for a very short lady
Fourth project: Raleigh Sprite 27" for myself Before After
Handlebars turned down, no brakes
Handlebars up, brakes. Very sensible

Here's mine - frame is an early '70s Flandria (not the Belgian one but the cheap version for the US market).
Buddy of mine at work welded in the second top tube - made to look like the old pre-1910's Iver Johnson. Paint is mint green two-component epoxy. NOS fifties era coaster brake which works like crap (yes I cleaned and regreased it). Cheap French leather saddle.

holds up fine. I still tend to rely on the brakes most but i often just back pedal up to stop lights. As a testament to the hub and the resiliance of old bikes, I raced this bike almost as pictured (gearing change and with clipless) in the Santa Cruz wildcat and won first fixed gear by 20-30 minutes.

this used to be my regular ride but i decided to make it a townie when i got my new bike, im going to repaint it and add fenders/ racks soon. also i need to find an inexpensive vintage looking seat for it.
